Our Mission

The mission of Tennessee Home Based Primary Care is to provide high quality, compassionate medical care to seniors and others that find traveling to a medical office challenging.

Integrated Care

Our approach is to listen to our patients and their caregivers. Once we understand the patient’s goals, we help create a plan that integrates all the medical, emotional, social, and care needs into support both during and in-between medical visits.

Dr. Miranda Huffman
                                                                                   Miranda M. Huffman, MD, MEd

Dr. Huffman is a family physician in Nashville, Tennessee. She is a graduate of the Northeast Ohio Medical University (NEOMED). During medical school, she took a leave of absence to pursue a Master’s in Education at Kent State University and a fellowship in medical education through NEOMED. She went on to complete her family medicine residency at the University of Michigan. She has served as family medicine faculty at the University of Missouri – Kansas City and Meharry Medical College in Nashville, Tennessee, prior to joining Western Reserve Medical Group in 2020. She continues to serve as adjunct clinical faculty at Meharry Medical College, where she mentors’ students in a free clinic and teaches clinical medicine.

Dr. Huffman loves the variety and long-term relationships that come with being a family physician. Her approach to patient care focuses on understanding her patients beyond just their list of diagnoses. She believes that medicine is a partnership between the physician and the patient to achieve mutually agreed-upon goals. Dr. Huffman thinks we are all capable of growth with the right support from the right team. She is certified as an HIV specialist and has a particular interest in the care of patients with substance use disorders and chronic pain. She lives with her husband, Jeremy, daughter, Betty, and pug, Koop. She enjoys cooking, reading, spending time in nature, and indoor cycling.

Our History

Dr. William (Bill) Mills originally founded Western Reserve Medical Group in 2007. The medical house calls group quickly became a leader in providing high-quality, home-based primary care to NE Ohio seniors. The group presented its care model and outcomes at many local, regional, and national meetings while growing and staying focused on providing compassionate, quality care. The group has grown outside of NE Ohio to service patients throughout Ohio and Kentucky, as well as Indiana (Indiana Home Based Primary Care), Virginia (Virginia Home Based Primary Care), and now has expanded to Tennessee under the name Tennessee Home Based Primary Care. With outstanding physicians, advanced practice clinicians, and clinical and administrative support staff, the group is delighted to partner with patients while they remain safely in their homes.
